The U.S. Coast Guard’s Office of Investigations and Casualty Analysis has released its report of investigation into the deadly July 5, 2023 fire aboard the Italian-flagged cargo vessel Grande Costa D’Avorio at Port Newark, New Jersey. The fire broke out while the vessel was conducting cargo operations at Berth 18 and burned for five days, ultimately resulting in the deaths of two Newark Fire Department firefighters and causing an estimated $23 million in damage to the ship. Due to the severity of the incident, the Coast Guard’s Northeast District commander convened a district formal investigation. The final report outlines systemic safety concerns and operational challenges encountered during the response, culminating in five safety recommendations, six administrative recommendations, five formal findings of concern, and the issuance of a safety alert.
